SWP Interest Form

We welcome applications from motivated individuals who are underemployed or low-income and looking for work in an office setting. Please fill out the Interest Form below if you are between the ages of 18 and 55.

StreetWise Partners defines "low-income" as being at 200% of the Poverty Line or below. The following chart shows the 2007 HHS Poverty Guideline.

  1. Please find the number of dependants in your Family Unit in the column on the left.
  2. Then, trace over to the right hand column, if your annual income is at or less than the figure there, please answer "Yes" to the question. If it is higher, please answer "No".
